Evidence, correction, and certification-body claim pathway
USDHS reviews material records under the same published methodology whether they raise or lower a Credibility Score. Submitters should identify the exact organization, article, factual statement, source, date, scope, responsible people, and requested correction.
An authorized representative may request to claim a certification body by supplying the exact legal entity, official website, organization email, title, authority basis, and qualifying authorization document. USDHS independently verifies authority. A name, title, email domain, employment, contracting relationship, or self-attestation alone is not verification.
The secure form accepts public source links and limited PDF or image attachments. It creates a private review record and reference number; it does not automatically change an article, Credibility Score, Grade, ranking, certification status, accreditation decision, or legal conclusion.
Contact information is used for evidence verification and follow-up only. This form does not create newsletter or campaign consent.
