Evidence and correction pathway
USDHS reviews material records under the same published methodology whether they raise or lower a score. Submitters should identify the exact organization, article, factual statement, source, date, scope, responsible people, and requested correction.
The secure form accepts public source links and limited PDF or image attachments. It creates a private review record and reference number; it does not automatically change an article, score, certification status, accreditation decision, or legal conclusion.
